Does it need to come out at all?
Not every wisdom tooth does. A healthy tooth with room to come through, which you can keep clean, can stay. Removal is usually advised when one of these is happening:
- Repeated pain or swelling in the gum over the tooth
- Food trapped behind it, with decay starting in the tooth or its neighbour
- A tooth stuck under the gum or bone at an angle, pressing on the next molar

Why the scan comes first
In the lower jaw, a nerve that gives feeling to the lip and chin runs close to the wisdom tooth roots. An OPG X-ray shows the general picture, and a CBCT scan shows the relationship in three dimensions when the roots look close. On the day
Most removals are done under local anaesthetic in the clinic's implant theatre, so the area is numb and you are awake. A tooth buried in bone may need a small cut in the gum, and it is sometimes taken out in sections so that less bone has to be removed. Stitches close the site if they are needed. You will know beforehand which of these applies to you.
The days afterwards
Swelling usually peaks around the second day and settles after that. Soft, cool food helps, and you should avoid smoking, straws and hard rinsing early on, because they can disturb the clot.
